Very frustrated. :(

I taught myself how to knit two years ago and I wanted to start up again. I read through the basics as a refresher course and when I tried it, it was nothing like I remembered. No matter what I did, once I started knitting, all the stitches were loose and uneven. I've taken it apart at least 10 times and still can't figure it out.

Can someone please help me possibly figure out what I'm doing wrong? (I really apologize if this was already asked.)
