Parisjem,

You do not need to hold the loop tight while doing your first round of crochet. You tighten the loop after you finish doing all your stitches. As to how to hold the loop -- try it out with hook and yarn in hand to see what is comfortable for you. I don't see why you wouldn't hold it the same way you would hold a ring of chained stitches that is traditionally used to start a center-out crochet pattern.

I do something similar to start a center out knit pattern. Emily Oecker first mentioned this technique for knitting and Elizabeth Zimmerman referenced in one of her books -- I think it was Knitter's Almanac.
