To add to KT's advice, if you have holes in the work as well as extra stitches, you are probably doing yarn-overs without realizing it. If you are just doing a straight knit stitch every row it's unlikely that unintentional yarn-overs are your problem. If you are doing a rib stitch (knit, purl, knit, purl...across), it is very likely that they are your problem.

The second situation that KT describes is very common for beginners and even seasoned knitters will sometimes do it if they are working very fast and are gabbing or otherwise distracted.
