Proper gauge trouble

I have recently started crocheting clothing. When I prepare a gauge sample, I find it never works out correctly no matter how I adjust my needles. I find that I have to use a smaller sized needle to obtain the proper stitch across (e.g., 16 sc=4") but I'm always too large on the height (10 dc rows=4"). What am I doing wrong? I'm using the exact yard each pattern calls for but my projects all come out too large. I then tried using the smaller pattern size (small instead of medium) and it comes out too small. :(
